Welcome to the SQG 2026 Quilt Show Entry Form

1. Please view, download and/or print the instructions and the Viewer's Choice Category Definitions so you can efficiently complete the form and have the necessary information for each quilt ready to enter.​ You can now enter up to 5 Quilts and 1 Chairman's Challenge Quilt per form.  You will receive an email receipt for each entry session.
